How to Register
By Mail
Return your completed registration form and a check made payable to UW-Madison
to Midwest Conference, Pyle Center, 702 Langdon St., Madison, WI 53706-1487.
By Phone
1-800-725-9692 or 608-265-3261 (7:00 am-4:30 pm central time). Call and charge
the registration fee to your VISA, Master Card or American Express account.
When you register by phone, we ask that before you call you have the registration
form completed, including your workshop choices, so you have at hand all the
information needed by the registrar.
By FAX
1-800-741-7416 or 608-265-3163. You can fax the registration form if you pay
by credit card or provide a purchase order number.
Online: Secure online registration is available for this program.
Note: You will need to know your workshop preference by number when you register online.
Please note: If you have trouble downloading the registration form, it may be that you need to first get a free copy of Adobe Acrobat Reader.
