    Return your completed registration form (204k pdf) and check made payable to UW-Madison to Eastern Conference, Pyle Center, 702 Langdon St., Madison, WI 53706-1487.
  • By FAX
    800-741-7416 or 608-265-3163
    You can FAX your registration form (204k pdf) if you pay by credit card or provide a purchase order number.
  • By Phone
    800-725-9692 or 608-265-3261 (7:00 am-4:30 pm Cst). Call and charge the registration fee to your VISA, Master Card, or American Express account. When you register by phone, we ask that before you call you have the registration form (204k pdf) completed, including your workshop choices, so you have at hand all the information needed by the registrar.

The Department of Professional Development & Applied Studies is a part of the University of Wisconsin-Madison Division of Continuing Studies. The units within Continuing Studies provide continuing education programs for lifelong learners, from precollege to seniors, as well as counseling services for adult learners.
