Be prepared to work as a care manager in the new integrated long-term care system.

The Case Management Certificate program is a continuing education training series for professionals in the field of aging and long-term care. There are two levels of certificates: introduction to case management and advanced case management. All seminars are open enrollment and are held at the Pyle Center on campus at the University of Wisconsin-Madison. This Case Management Certificate series in aging and long-term care is developed and coordinated by Suzanna Waters Castillo, PhD, Faculty Associate for the Department of Professional Development and Applied Studies (PDAS), scastillo@dcs.wisc.edu.

"Each Social Work Case Manager shall assume personal responsibility for continuing professional education. He/she should complete 90 hours of such education every three years in accordance with the NASW standards for continuing professional education."

Please note:  Anyone who wishes to can take this Case Management Certificate Series. It is a skills-based program to further your knowledge of case management practices for aging clients in long-term care situations.  However this does not "certify" you to become a case manager.

  1. You will learn the latest research and strategies for recognizing and treating the problems that your clients are facing.

  2. You will learn to assist clients in overcoming barriers that interfere with medication regimen adherence when health literacy and cultural factors may be a problem.

  3. You will learn to identify various types of depression related to late life mental health and intervention techniques to match treatment with client needs and characteristics.

  4. You will learn research and breakthroughs in dementia and Alzheimer's diagnosis and treatment and current treatment.

  5. You will learn, as a professional, how to assess and effectively respond to the emotional distress of clients with dementia and how to provide person-centered care that attends to the strengths and abilities of clients with dementia.

Advantages of the certificate series:

  1. 20 continuing education hours: Introduction to Care Management
  2. 25 continuing education hours: Advanced Care Management Certificate
  3. State of Wisconsin NASW continuing education hours
  4. Acess to professional consultation
  5. Skills that are applicable for working in the new integrated integrated care management system.

The following 4 criteria are used for Advanced Case Management: Certificate II. They are cited from the text "Advanced Case Management" by Raiff and Shore.

  2. Multidisciplinary Practice
  3. Consumer Empowerment
  4. Cultural Competency

Upon completion of either of the Case Management Certificate series, you will receive:

Introduction to Care Management: Certificate I

Certificate I must be completed over a two-year period.  After Certificate I is completed, you will then be eligible to proceed to the Care Management Certificate II series of classes.

Total CEUs: 20 hours

Advanced Case Management: Certificate II

Pre-requisite: Introduction to Care Management: Certificate I

Total CEUs: 25 hours

Certificate II is completed over a two-year period. The core 25 CEUs for the Advanced Case Management Certificate II must include seminars that fit the following advanced practice care management categories. Coming Soon – Professional Certificate Program on Mental Health and the Older Adult:

This certificate program begins with the July 2008 Summer Institute on Mental Health and the Older Adult.  The topic of this two-day institute is:  Fundamental Assessment and Treatment of Mental Disorders in the Older Adult.  This advanced training program is for licensed practitioners and allied health and human service professionals that work with older adults.
