St. Mary's Minority Health Care Scholarship Program, Scholarship Information
St. Marys Hospital Medical Center Minority Health Care Scholarship Program
is available to provide financial assistance to individuals who are interested
in and committed to pursuing a health care profession. Scholarships are
awarded to two minority students who meet these requirements.

Accepted or planned enrollment with University of Wisconsin-Madison,
Edgewood College for the Bachelor of Science Degree, or Madison Area
Technical College for an Associate Degree. Course requirements should
apply towards admissions to one of the following programs:
Dietetics
Nursing
Physical Therapy
Medical Laboratory Technician
Occupational Therapy
Radiography
Medical Technology
Pharmacy
Respiratory Care

Hold high school degree or equivalence requirement by June of the
year application is submitted.

Hold a grade point average of 2.5 on a 4.0 scale or meet the high
school equivalence requirements.

Must be entering first, second, or third year in a program without
previous experience in a professional working position.

Provide evidence of U.S. citizenship or documentation of permanent
residency status.

Hold membership in a minority group as defined by the U.S. Equal
Employment Opportunity Commission (African American, Hispanic, Asian
or Pacific Islander, or Native American.)
Application and selection procedure

Submit a completed application form including all current transcripts
by March 1. Please notify the individuals selected for recommendations
that information will be mailed to them for your application. Preferably
two recommendations should be from current or former instructors.

Two scholarships are awarded annually in May for the following fall
semester. After applications are reviewed, applicants will be selected
for an interview.
Scholarship provisions

The scholarships provide $1,250 for tuition each semester paid directly
to the school (up to $2,500 per academic year). The scholarship will
be available up to four years with annual evaluations within the admission
guidelines of the chosen program.

Every two weeks recipients will be provided with 16 hours of paid
employment. Employment in an assistant position provides valuable
related work experience.

The scholarship may be renewed each year provided:

Cumulative 2.5 grade point average is maintained with enrolled
in at least nine hours per semester.
NOTE: A one- semester grace period is applied for cumulative average
below 2.5.

Employment of 16 hours every two weeks with acceptable performance
and attendance as a regular employee is maintained.
